Please complete this form if you are interested in connecting with one of our staff about programs in nutrition, food safety, physical activity, financial wellness, and mental well-being.  Please review our website to see a list of current programs and direct contact information to see if any may best suit your needs. We ask that you please provide dates that you are interested in having us attend at least 6 weeks in advance to allow for ample planning time for our staff and volunteers. We will make every effort to accommodate each requested program, day and time, but please recognize that sometimes our schedule may not allow us to do so.
