Fill this form out if you would like to use money from your Fine Arts Booster Student Account towards an item, activity, trip, etc. Before completing your form, please check your Student Account balance using Charms Office Assistant.

After filling out the form, you will receive an email copy of this form for your records. Upon submitting this form, it will also send an email notification to the Fine Arts Boosters. Once the request has been processed, the boosters will email back if your request has been approved or declined based on the funds in your account.
