Dance Camp REMIX Summer Camp Registration 2022
Camp Times: 11:00 a.m.-1:00 p.m.
Camp Ages: 7-11 years old
Camp Dancer Maximum: 15 dancers
Camp Fees: 3-days- $100/ 4-days- $125
**Be sure to check off all days your dancer will attend.

Camp Dress Code: Leotard and tights are welcome but not required. Athletic wear is approved. If dancers do not have needed shoes they can dance barefoot or may borrow shoes provided by the studio if available.

*To reserve your spot you will have to fill out this form and call or send in 1/2 or your full camp tuition at the time of registration. New students to our dance studio will need to provide more information, you will be emailed personally after registration.

*Dancers will need to bring bottled water with them each day.

*Registration will close on May 31st or if a camp reaches capacity

If needed our electronic Covid-19 waiver will be emailed 1 week before camp and is due before the first day of camp. I understand that if a waiver is needed and I do not complete this waiver my dancer will not be able to participate in camp until the waiver is completed. *

In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and/or all claims of negligence arising as a result of such activity from which liability could accrue to Prestige Dance Studio, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities.  I further agree to release and hold Prestige Dance Studio harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility, for all risks of physical injury arising out of active participation in the activity of dance.  I acknowledge this is a material inducement to Prestige Dance Studio allowing me or my child to participate. *
