This form is for registration. It asks for basic information on the number of people attending from your family. If you do not intend to participate in a particular field trip, you can just skip the page (no need to put "0" attending). The field trip descriptions are not included on this document. If a field trip on the description page is not listed on the registration form, it is because the field trip is full at this time. If the field trip reopens due to a family cancelling or an increase in the number of people who can attend, a note will go out to the CHDH Google Group letting you know registration has reopened for that field trip.

After completing this form, you will receive a confirmation. It will include your responses, including the amount you need to pay for each trip. If you have selected any field trips that require payment, your registration will not be considered complete until payment is received. You can pay for all the field trips in one step (no need to pay for each trip individually). Just add up your total based on your responses to the individual trips and pay in one payment. You do not need a Paypal account to pay through Paypal. The confirmation email will contain this message, including this link to the CHDH Paypal account:
PLEASE PUT YOUR NAME IN THE NOTE SECTION OF PAYPAL SO WE CAN MATCH YOUR PAYMENT WITH YOUR REGISTRATION.
The confirmation email will also include a link to edit your responses. Please save the email in case you need to change your responses or register for additional field trips.
CHDH will not be able to issue refunds if you are unable to go on a trip for which you have registered and paid. Many of the trips have a minimum number of participants required for us to receive the group rate, so making changes after we have confirmed is not an option.
