Brooklyn Collaborative Studies became a Community School In Summer of 2022. As a Community School, we have partnered with the organization Inspiring Minds NYC to provide after school programming, elective classes, college & career readiness, internships, and support services to our students. They will also be providing programs and workshops for our families, as well as our staff.

This enrollment form is required in order to be able to attend any services or programs that Inspiring Minds NYC provides at BCS. We ask that all families complete this form before the programs begin in February so that we don't have to turn away any students who may want to participate in a Lunch, Trip, or After School club/program.

This form is currently required to be filled out "ONCE" for the entirety of your child's time as a student at BCS.

The enrollment form will only be truly complete when you click the Submit button at the end of the form. There are 6 sections/pages:

Student and Parent/Guardian Information
Dismissal and Pick-Up Information
Consent for Photograph/Video Release
Health Information (Optional)
Student Demographic Information (Optional section)
Evaluation Information and Opt-Out (Optional)

Our school's Community School Director, Ashley Armand , will contact you to confirm your form was completed and verify your approval for your student's participation.
