Registration is a 2 part process.
Step 1. By completing and submitting this form, you are confirming your spot is reserved for the training you select. Please note each individual attendee needs to fill out their own form, in order for their spot to be held.
Step 2. Payment - If you plan to pay with a purchase order and need an official estimate, please indicate this request on page 4. We will then contact you within 5 business days with an official estimate to submit to your purchasing dept. All PO's should be sent to firstname.lastname@example.org. If you plan to pay with a credit card, we will contact you within 5 days of submitting this form - At that point, you will be provided an official invoice, and payment instructions. Please note, If we have not received a PO or pre-payment within 30 days of the training date, your spot will be released.
