SWBOCES complaint procedure form for breaches or unauthorized releases of protected data
The regulations of Education Law 2D requires that educational agencies establish and disseminate procedures for parents, eligible students (students who are at least 18 years of age or attending a postsecondary
institution at any age), principals, teachers, and employees of an educational agency to report possible breaches or improper disclosure of student data and/or protected teacher or principal data.

Please fill out the following form with as much detail as possible. As per regulations, all reported claims will be investigated and findings reported within sixty days of the reported event.
