Please use this form to request the release of your FMHS student.
Once submitted, this form will be automatically sent to the attendance department. 

Please allow thirty (30) minutes, possibly more, for the release of the student driver from the time of submission.

   District Policy states students will not be released within the last 30 minutes of the regular dismissal time.

1. It is recommended that electronic sign outs be inputted prior to 11am, even for a later dismissal time.
    This will give our staff time to call and verify the sign out.
2. The Electronic Sign-out form must be used, email requests will not be reviewed.
3. This form is for student drivers ONLY, any non-drivers would need to be signed out in-person via main office       by a parent or guardian.
4. A separate form must be completed for individual students, even if they will be leaving in the same vehicle.

Parent/Guardian will be contacted by the school to confirm the electronic sign out request. The phone number given must match the parent/guardian information currently on file in FOCUS. If it does not, your child must be signed out in person, with proper identification.
