SVHS Proctor Volunteer Application
Volunteers are always appreciated at South View High School. Thank you for your interest and I hope that you will be able to help us out.

• Proctor Training will be given by Ms. Tyson Johnson and Mrs. Samantha Sheppard on Monday, December 10, 2018 from 9am – 10am and on Tuesday, December 11, 2018 from 4pm – 5pm in the SVHS Media Center.
• Please park in the Student Safari Parking lot that is closest to Elk Road on testing day.
• Please be prepared to arrive by 8:00 AM and stay at least until Noon each day, or until testing is completed.
• Please wear comfortable, soft-soled shoes when proctoring and be prepared to turn off all electronic devices.
• Parents/Guardians of seniors have the opportunity to earn graduation tickets by proctoring for exams. The first 25 proctors who turn in this form and show up to proctor for any date during December 17, 2018 – December 21, 2018 will earn 1 ticket per day they proctor. After the first 25 applications have been submitted, additional applications will earn 1 ticket per every 2 days that the applicant participates.

Proctoring Dates *
Please check ALL dates that you are available to proctor for us. Time frames are from 8:00 AM until roughly 12:30 PM. Date selections should be based on what your availability is. We appreciate the time you give us.
Training Date *
All volunteers are required to attend a training session to familiarize themselves with the rules and regulations of state testing. Please choose a training session listed below to attend. If you are unable to attend one of these dates, please contact Mrs. Sheppard as soon as possible (preferably via email) so that she can work out another option for you to be trained.
