Please read the following information carefully and complete this form if your pet needs to receive vaccinations, heartworm testing, felv/fiv testing, deworming, or other basic preventive services offered by Prevent A Litter. You will receive a follow-up call to confirm details and to schedule an appointment. Due to the volume of inquiries we receive, please expect that IT MAY TAKE UP TO TWO-FOUR WEEKS TO RECEIVE A RETURN PHONE CALL. Please only submit ONE form, even if you have multiple pets. Submitting multiple requests increases our response time.
If you don't remember what specific vaccines your pet needs, that's okay. Please do not call to ask. We will go over that information when we call you back to schedule an appointment. Any animal receiving vaccines or services at PAL must either be up to date on their rabies vaccine, or must receive the rabies vaccine while at PAL, as it is a legally required vaccine (does not apply to pets under 12 weeks of age/less than 3 lbs). Veterinary records confirming your pet is up to date on their rabies vaccine must be provided at your scheduled appointment, prior to receiving services.

PRE-PAYMENT FOR SERVICES AND PRE-SCHEDULING IS REQUIRED FOR ALL APPOINTMENTS!
FOR ANY VACCINE APPOINTMENT CANCELED WITHIN 48 HOURS OR LESS OF THE SCHEDULED APPOINTMENT DATE, THERE WILL BE A $15 CANCELATION FEE OWED PER PET, PRIOR TO RECEIVING ANY FUTURE SERVICES AT PAL. IF AN APPOINTMENT NEEDS TO BE RESCHEDULED, WE CAN RESCHEDULE ONCE FOR FREE. IF RESCHEDULING IS NEEDED AGAIN AFTER THAT, THEN A $15 RESCHEDULING FEE WILL BE OWED PER PET, PRIOR TO RESCHEDULING THE APPOINTMENT. PLEASE NOTE, THE CANCELATION FEE AND RESCHEDULING FEE ARE TWO SEPARATE FEES.
