IMPACT EVALUATION SURVEY ON PRODUCTIVITY BASED PAY

a. This survey questionnaire is intended to assess the impact of PRODUCTIVITY BASED-PAY as a wage policy implemented by the Regional Tripartite Wages and Productivity Board IV-A thru Wage Order No.IVA-15.

b. We propose that this survey form be accomplished by management and workers' representatives who have full knowledge of the company's Productivity Based Pay, Performance Based Pay or Profit Based Pay, Productivity Improvement and Incentives Committee or equivalent structure and Productivity Improvement Programs.

c. We guarantee confidentiality of your responses which will be used solely for assessing the impact of Wage Order IVA-15 (which became effective May 15, 2012) and in the formulation of future wages policies.

d. This survey is conducted in collaboration with Lyceum of the Philippines-Laguna, People Management Association of the Philippines-CALABARZON, Employer's organizations, HR organizations, Industry Associations, Labor organizations, PEZA Administration Offices, DOLE-IVA Provincial Offices, and selected LGUs.
