Final Instructions: A copy of your form submission will be emailed to the address you provide in this form.
Refund requests must be in writing to Cathy Keagy:
- By March 31, 2025 (email date or postmark) - 100% less $20 processing fee
- By April 30, 2025 (email date or postmark) - 50% less $20 processing fee
- NO REFUNDS after April 30, 2025
