Youth Survey
Santa Cruz County youth ages 11-18, we want to hear about your experience at the Capitola Mall and Capitola Village! What do you enjoy? What do you want to see more of? Your input will guide us in creating safe, engaging, and fun spaces for youth.

This survey is anonymous. If you'd like to participate in our raffle or leadership opportunities, we ask that you include your contact info. 

This survey was created in collaboration with Capitola City, Capitola City Police, United Way of Santa Cruz County- Youth Action Network, Cradle To Career, and the Santa Cruz County Office of Education. 

Survey closes June 17th. Raffle prize winners will be contacted the week of June 19th.
