Hillcrest families please use this form to:
*receive a your two free complimentary car tags if you are NEW to Hillcrest
*you need to PURCHASE extra tags ($5 per extra tag-cash only)

KINDERGARTEN PARENTS: you will have an opportunity for pick up during k days, this form will speed up your time getting tags. If you already have a child at Hillcrest, please stop by the office and let Ms. Jordan know to add your kindergartener to the car tag assigned to your other child(ren) or fill out this form.

Once form is filled out, Ms. Jordan from our Hillcrest office will contact you to come pick them up from the office.
If you have been issued your 2 free tags, each tag after that is $5 (cash only).
