Please submit requests for Open IBD blood and biopsy collection consumables using this form. These items are provided by the Sanger Open IBD laboratory in Cambridge; if you require stool packs please follow this link or contact the team in Newcastle.

Please help us to accommodate busy periods by allowing up to two weeks for delivery. As we keep limited stocks in the labs, please request quantities that are appropriate to your usual recruitment rate. You will receive a copy of this form upon submission and lab staff will dispatch your request as soon as possible.

Any information provided here will be used for fulfilling consumables requests only. Where necessary contact information will be shared with the courier to enable successful delivery.
