CPCS Student Transportation & Communication Form 2020-2021
Whether you are choosing the 100% virtual learning -OR- the hybrid (2 days in-person instruction) option for school this year please determine your child/children's transportation plan and fill in the appropriate options below. If your child lives in two households, please fill out based on your individual transportation needs. Use the 'Additional Notes' section at the end of this survey to define your schedule if necessary. If you expect your child to ride a school bus, please refer to the CPCS District Routes and determine the bus number stop name prior to completing this form.
