Holiday Assistance Sign-Up

This is a form to sign up for Holiday Assistance for students enrolled in Haywood County Public Schools. You will fill out one form per child. We will take a limited number of 0-4 year olds who are not old enough to be enrolled in school.  Read all questions carefully and respond as appropriate.

Some programs provide toy items from a wish list. Resources are limited, therefore, your child may not receive what you list as their wish items. When filling out clothing sizes, be as specific as possible (infant, junior's, men's, petite, tall, etc).

 Please be aware that no applications are to be turned into your child’s School counselor or Social Worker this year. Also, due to the number of families needing assistance, families can only sign up with one agency for assistance. Due to the limited number of funds we cannot guarantee that you will receive Christmas assistance. 

Filling out and submitting this form DOES NOT guarantee assistance. Once all form responses are collected, your child's school will contact you to let you know if your child has been selected to participate in a program.

Once you have clicked "Submit" at the end of the form, you should receive an email confirmation that you have applied.
