Princess Camp!

It will be a royally good time for your little dancer! Your child will experience joy and excitement with crafts, games and a whole lot of imagination! A snack will be provided. Along with all the fun and play, your dancer will also be learning the basics of ballet and jazz. Space is limited!

This camp is for dancers ages 4-6. Camp is Tuesday-Thursday August 20th-22nd from 1-3pm. Cost is $135 for the week. A $35 deposit is due upon sign up. Remaining balance will be charged August 1st.

I understand that I will be charged a $35 non refundable deposit to hold my child's spot in the camp at the time this form is submitted. Additionally I agree to allow SDA to charge my card on file $100 for the remaining balance on August 1st. Payment is non refundable after this date.

By checking this box and submitting this form, I understand (the parent/guardian listed above) that I am signing up my child/children to participate in an event under the supervision of Sherwood Dance Academy (SDA) staff.  I understand that it is the express intent of all SDA staff to provide for the safety and protection of my child/children.  In consideration for allowing my child/children to participate, it is agreed that I waive all rights and claims for damages against SDA and SDA staff for any injuries or damages suffered during this event.
