Please complete the following form. Upon completion your payment can be made by either of the following methods:

1) Using PayPal via the following link: www.indianaaged.org
* Winter Inservice Registration will be $50
* Please note that there will be a 3% service fee added for all Paypal transactions

2) Include with Winter Inservice Registration for a total of $135 (Paid in December/January)
* Non-member Winter Inservice Registration will be $150

3) Mail a check to the IAAE Treasurer at:
