FORM INSTRUCTIONS:

- The Parent/Student form is due by May 2nd. *10th Grade PTECH Students must have the application completed by January 17th.

- The Counselor's portion of the application is due by May 16th. *For 10th grade PTECH students, please have the counselor portion completed by February 28th.

STEP ONE:
Students must complete the following application in its entirety. You will need your SCHOOL COUNSELOR'S NAME, EMAIL AND PHONE #.

Once complete, CLICK SUBMIT at the end of the form. The student, parent, and home school counselor should receive a notification that the student portion of the application has been submitted. Please check your spam for this response.

*PLEASE USE CAPITALIZATION AND PROPER PUNCTUATION*

For accepted students, data from this form is used for operating systems.

STEP TWO:
Student should contact their home school school counselor and ask them to complete the COUNSELOR APPLICATION located at: https://forms.gle/hxatQ8xpKveCYmeg6
