Form Instructions - Student form due by May 1st. Counselor's part due by May 15th

STEP ONE:
Students must complete the following application in its entirety. You will need your SCHOOL COUNSELOR'S NAME, EMAIL AND PHONE #.

Once complete, CLICK SUBMIT at the end of the form. The student, parent, and home school counselor will receive a notification that the student portion of the application has been submitted.

STEP TWO:
Student should contact their home school school counselor and ask them to complete the COUNSELOR APPLICATION located at: https://forms.gle/hxatQ8xpKveCYmeg6

!! Student portion of the application must be completed by May 1st.

!! Counselor applications must be completed and submitted electronically via the Google Form by May 15th

!! AN APPLICATION IS NOT COMPLETE UNLESS BOTH THE STUDENT/PARENT AND SCHOOL COUNSELOR PORTION HAVE BEEN RECEIVED!!
