ARHS Parent Dismissal Form
If you intend to dismiss any student prior to 1:48 PM on Monday, Tuesday, Thursday, or Friday or prior to 12:48 PM on Wednesdays, you must complete this form. A form must be completed for each student and for each instance.

This form needs to be completed by 8:00 AM on the day of the dismissal.  If dismissing for a doctor's appointment or court appointment, please make sure to receive a note about the appointment. The student should bring that note to the Main Office upon return.

Please enter the parent/guardian email below. This must be sent from the official parent/guardian email contact on file at Apponequet Regional High School in order to be a valid dismissal.  

This form is not necessary for grade 12 Senior Privileges departure.

Reminder - students must be present for a minimum of 5 periods to participate in ANY after school activity unless documentation is provided such as doctor's note, note from court, etc.
