COMMUNITY SERVICE AWARD CRITERIA

This award is presented to an individual, over the age of 18, who:

• Made or is making significant contributions to the welfare of racetrack chaplaincy ministries through involvement, participation; and

• Gave freely of their time and energy for the betterment of racetrack chaplaincy ministries, contributing to positive ministry outcomes in the horse racing industry

NOMINATION FORM

The nominator must provide contact information for the nominee and a detailed description of the basis for the nomination. Eyewitness accounts, testimonies, and any other supporting information are encouraged.
 
RTCA staff is not eligible.
