Please fill out the required information below. You may fill out as little as one name or as many as four to fill your foursome. Please coordinate with your group so multiple entries are not submitted. If you are a single, twosome or threesome, write your name and we will pair you up with others to create a team of four.

Power Carts are for Seniors with a medical condition only and will be given out on a first come first served basis on the day of.

Entries must be paid to secure your spot or you will be removed. Payment options are as follows:

1. E-Transfer to plorimer@sympatico.ca (password: golf) - preferred method
2. Cash paid to Matthew (our GM) or Steve Vale in the office at Centennial

These payment methods allow up to donate 100% of the proceeds to Women's Habitat.
