I understand that two unexcused absences will result in loss of placement in the program.  An invitation to register for the following term will be extended. 

Building Brilliance KW and Kingsdale Community Centre are not responsible for lost, stolen or damaged personal articles.

I authorize Building Brilliance KW to have and use photographs, slides, videos and comments on the person(s) named on this application needed in promotional materials and public relations programming.  

I individually and corporately agree to hold harmless, Building Brilliance KW and Kingsdale Community Centre and/or those individuals arising from or related in any way to this Building Brilliance KW Tutoring Program 2026.  I understand that I will provide a permission signature upon arrival and departure of the program.
