Due to the NCRHA Divisional Restructure, announced on July 20, all teams need to re-submit their Divisional Application for the 2018-2019 season. Please make sure you have read the article and read the Divisions Defined section of the League Operations Manual before completing this form.

This form needs only be completed once per club. Responses are due by July 29, 2018 at 11:00PM Eastern.
Applications must be completed every season in which the club intends to participate.
1. This form is an application for divisional placement. Some applications may be denied, in which case the team representative can appeal the decision, per the process outlined in step #3.
2. The NCRHA and Conferences will issue decisions on applications no later than August 4.
3. For teams whose applications are denied, all appeals are to be submitted by August 15. Appeals are to be emailed to the NCRHA at firstname.lastname@example.org, along with the original application as well as any other information that supports the appeal.
4. Final rulings will be issued by the NCRHA no later than August 30.
It is the responsibility of the team representative to provide another responsible team member’s contact information if the team representative is going to be unavailable during these dates.
