Transfer request form
Please complete this form for transfer requests for the 2022-23 school year.  

How the process works:  Families who request a transfer will be placed into a lottery.  Requests received by July 15th will be eligible for the transfer lottery.  Transfer requests will be honored based on space availability and will be a weighted lottery.  Requests received after July 15th will be honored in the order they are received.  Placements from the transfer lottery will continue until the 20th day of school.  If your child is unable to be transferred this school year, you may request a transfer during the transfer period in the spring for the 2023-2024 school year.  

Only one transfer per year is permitted.

All transfer requests must be received by submitting this form.  The Family Resource Center cannot accept transfer requests over the phone or by email.
