Incident Intake Form 
The purpose of this form is to report any FinOps Foundation Code of Conduct violations to the Code of Conduct Committee (CoCC). The FinOps Foundation's goal is to ensure that you can communicate issues and concerns associated with the Code of Conduct safely and honestly with the CoCC.  The committee strives to respond quickly to reports, as well as initiate whatever actions are appropriate based on severity, risk, urgency, and impact. If more information is needed during the review the CoCC will contact the reporter.
