Senior pitch bookings during November are available for Tuesday 3rd November ONLY.

Junior training is available on Monday 2nd November for U14/U16 ONLY.

Tuesday 3rd November:

Session 1: 19:00 - 20:25 (W1)
Session 2: 19:00 - 20:25 (W2/W3)
Session 3: 20:35 - 22:00 (M1)
Session 4: 20:35 - 22:00 (M2/3)

Based on the allocation of training times, we do not anticipate any session to exceed a group size of 30. Please only attend the sessions designated for your team, as we will no longer be sending out booking confirmation emails. 

Sessions will still be allocated on a first come first served basis, and in the event  that a session is over subscribed, we will send out emails to anyone who is unable to attend.

All members will have to agree to the Covid-19 Operating Procedure for use of the New Field facility (at the bottom of this form) AND the England Hockey GDPR statement.

Covid-19 Operating Procedure
Members can only attend sessions if they have booked via the appropriate google form

Members must not attend any sessions if they have any symptoms of Covid -19 as outlined on the NHS website

Members must not attend any sessions if they have been informed via track and trace that they need to self isolate

Members must inform the Club via whhcbookings@gmail.com if they have a confirmed positive test for Covid-19

Members must arrive for their session no more than 15 minutes prior to the scheduled start time - and stay in their vehicle until the previous session has cleared the car park - or informed to do so by the coach

Parents and guardians are encouraged to remain inside their vehicles if waiting to pick up their child

Spectators must remain outside of the fenced area at all times and are encouraged to maintain social distancing

Players must enter the pitch via the left hand gate (nearest the car park) and maintain social distancing guidance

Players can place their kit and water bottle along the fence line next to the access road

Players are not permitted to touch any of the equipment by hand (balls/ cones)

Players must exit the pitch via the right hand gate (furthest from car park) and vacate the site promptly

Players may access the disabled toilet facility in the main atrium of the pavilion and follow the one way system

There is strictly no access to changing rooms or the upstairs of the pavilion

Purpose of data collection
The data is collected in order to support NHS Track and Trace. This information will only be used where necessary to help stop the spread of COVID-19.

Legitimate interest or public task

NB In order to collect personal information based on legitimate interest, a legitimate interest assessment must be conducted. This requires the following to be assessed:
- The purpose of data collection
- The necessity of data processing
- The balance between the rights of the data subject and the data recipient in collecting and processing the data

Data storage
- Data will be stored securely in accordance with the GDPR

Data retention and deletion
Attendance Records: To support NHS Test and Trace, attendance records will be retained for 21 days from receipt. This reflects the incubation period for COVID-19 (which can be up to 14 days), an additional 7 days to allow time for testing and tracing and allows for reporting time. After 21 days, this information will be securely disposed of or deleted.
Participation Agreements: These will be retained for a period of one season and renewed if required. This is to ensure that participants can be contacted by NHS Track and Trace and/or appropriate Covid Officers if necessary.

Data sharing
"In the event that the Covid Officer is contacted by the NHS track and trace, the details you provide will be shared. NHS Test and Trace will ask for these records only where it is necessary, either because someone who has tested positive for COVID-19 has listed this activity as an event in which they participated, or because this event has been identified as the location of a potential local outbreak of COVID-19. For more information on the use of your data by NHS Track and Trace please visit the .gov website (https://www.gov.uk/guidance/nhs-test-and-trace-how-it-works)

The data will also be shared with England Hockey for the purposes of supporting NHS Track and Trace."
