OTPS Grab and Go Meals
This form will serve as your order for breakfast/lunch Grab and Go Meals. Parents must complete this form to place an order. Orders must be received by 9am the day prior to pick up. For Monday pick up the form must be received by 9am the Friday before to allow our staff to prepare for all incoming orders. Please submit one form per family. Meals can be picked up at the Upper Elementary storm shelter 9:15am-9:45am.
