AYSO REGION 65 - Player Drop Form

Please fill out this form below if you wish to drop/cancel your registration for the season.

As stated in our registration process and our Refund Policy online, a refund before deadline for cancelled registration. The refund is prorated for uniform, pictures & number of weeks since the season has started.  The $25.00 AYSO Fee is Non-Refundable.

Please fill out the request form and when your registration is cancelled you will receive an email when the cancellation is complete.
