HCC 2021-2022 New Student Registration Packet
TO BE INCLUDED IN THE 2021-2022 LOTTERY YOUR REGISTRATION FORM, AND THE REQUIRED DOCUMENTS BELOW, MUST BE RECEIVED NO LATER THAN FRIDAY, MARCH 5, 2021 AT 5:00pm (PST).

All students new to Hayden Canyon Charter are required to show valid proof of residence, a valid Birth Certificate and current Immunization Record (or valid immunization waiver) before they will be allowed to attend the school. To complete your application, please send the above documents to either the school 13590 N Government Way, Hayden Idaho 83835 or email to registrations@haydencanyoncharter.org.

According to the state, these are acceptable forms for proof of residence:

DRIVER'S LICENSE IS NOT CONSIDERED VALID PROOF OF RESIDENCE AND CANNOT BE ACCEPTED.
Lease/rental agreement, mortgage or deed
Account statement from one or two different public utilities. Must show service address of residency and customer's name; 3⁄4 statement page accepted. Cell phone bills are not accepted.
Account statement from bank or financial institution
Tax document Form W-2 or Form-1099 (must have legal name)
Medical or insurance provider statement, invoice, or explanation of benefits
Pay stub or employment verification (must show legal name)
Motel, hotel, campground or RV park receipt on letterhead that indicates 30 days or more of residency
Court documents (original or non-altered, and must have current address on the document for proof of Idaho residency)
Government or official correspondence to the residential address (must have legal name)
