2019 SLO CSA Contract Form
SLO is a cooperative run and owned by small farmers in Northeast Wisconsin. We are working together to market our Sustainable, Local, and Organic food to our community. We are excited to offer our first vegetable CSA starting this season, and it will be customizable! This means that you can swap out veggies on our online platform, add any additions for a given week, and pause your account any time for vacation.

Full Circle Community Farm (Green Bay/Appleton) and Flying Tractor Farm (Door/Kewaunee Counties) are the two main farms working together in 2019 to provide you with the fresh, high quality vegetables, herbs, and fruit. The farm closest to your pickup or delivery location is your partner farm, through whom you will be getting the majority of your vegetables. They will be the ones at your pick-up site and should be who you contact with related issues.

Email will be our primary mode of communication. We will send out exact dates and pickup locations as the time draws closer and we get an idea of where a lot of our members live. Right now possible pick-up locations are Lineville ave. in Howard, the Green Bay Botanical Gardens, Kress Family Library in DePere, World Headquarters in Appleton, Full Circle Community Farm, and Flying Tractor Farm. General weekly pick-up times are Wednesday and Thursday evenings. Additional pick-up sites are possible if we have enough members picking up (6 minimum). Let us know if you would like to help arrange a pickup at a home, office or other business. We will also be offering home delivery at an additional $7/week charge.

Our summer season is 20 weeks from mid-June to mid October with two size share options: standard share ($700; $35/week) or a small share ($500; $25/week), with an optional egg share add-on ($90; $4.50/dozen/week).

Filling out this form and sending in payment makes you officially signed up for a share of SLO Farmer's Co-op 2019 CSA season! We are asking for a $200 up-front payment by check, or credit card ($5 cc processing fee) on our website: www.slofarmersco-op.com

As a CSA Member, you are truly a shareholder in your local food economy: you connect with your community, support local farmers, and voice your right to have good, healthy food. Please read the statement below: *
Our primary goal as CSA farmers is to provide you with the highest quality, organic produce we can grow. We will try our best to make your share as valuable to you as your membership is to us! Many circumstances are beyond our control, and in the event of a partial or complete crop failure from weather or pests, we may fill your share with other crops grown on a SLO member farm that are ready for harvest at that time. This may not be a feasible option for all crops because of cost or widespread failure. Weekly boxes may vary somewhat in size, but should average out to being at or above market value.
Please read the following regarding your responsibilities as a member: *
• CSA membership is a season long commitment. If you can’t complete the season as members, for whatever reason, you are welcome to reassign your share to another household, but in general we don’t issue refunds for shares. • You are responsible for picking up your share. If you are unable to pick up your share on delivery day, please make alternate arrangements for your share ahead of time by asking someone else to pick up for you or select online to skip pickup for that week. If you forget to pick up your share on delivery day, it will be donated to a food pantry or neighborhood family. • Keep a copy of your site host's contact info. • Read the newsletters and emails from the farm. You'll find important announcements, recipes, and news from the farm.
A copy of your responses will be emailed to the address you provided.
