The mid-year report will also assist CHNA 20 in assessing where grant recipients are in their project cycles and what progress has been made towards the use of the grant funds. It will alert us to any potential challenges or roadblocks that may be hindering efforts, so we can appropriately shape our technical assistance for the grant recipient group.
When completing this report please refer to the information you provided in your initial grant application and Theory of Change. Note that this report includes three parts:
Part 1: Project progress summary (contained in this survey form)
Part 2: Results accountability matrices (provided via email)
Part 3: Budget accountability form (provided via email)
This form, and the additional documents sent by email, are due no later than November 30, 2015.
