Yearbook Order Form

Please fill out this form to order your WFS 2022-2023 yearbook.

**Complete a SEPARATE form for EACH yearbook ordered**

Again, ALL 5th grade students will receive ONE yearbook for free. You DO NOT need to fill out this form if your child is in 5th grade

!!*PLEASE NOTE*!!

If you are ordering a yearbook after the original deadline of 6/5/23 - more yearbooks had to be ordered and are currently being printed. This means that your ordered yearbook might not arrive until AFTER the school year has ended. You will be contacted as soon as your order arrives. Arrangements will be made for you to pick-up your yearbook.

-Quantities are limited

Must be received immediately following order being placed. Checks and cash will be unable to be collected once school has closed for the year. If possible, please use Venmo to ensure your payment has been made and received quickly

Cash - Exact amount, no change will be give. Send to school in an envelope marked: "Yearbook - child's name/teacher" -PAYMENT NEEDS TO BE SENT INTO SCHOOL THE DAY AFTER ORDER IS PLACED

Venmo (please send payment(s) to @WFSPTO) - PAYMENT MUST BE MADE AS SOON AS ORDER IS PLACED

Check -please make check payable to: WFS PTO and send into school in an envelope marked with: student name, teacher name and ”Yearbook" - PAYMENT NEEDS TO BE SENT INTO SCHOOL THE DAY AFTER ORDER IS PLACED
