*Use this form to register and pay for students attending from your school. You (or the students) will complete the COA West Student Registration Form (link provided after payment) to make workshop choices.
Some Things to Note
Lunch is not included with the registration fee.
Students are encouraged to bring a sack lunch.

Cavalcade of Authors West may be attended by media organizations and the students in attendance may be recorded, photographed or have other content taken by media organizations invited by CoA West. Throughout the event CoA West will be taking photos of students engaged in workshops and Cavalcade activities. We intend to use these photos on our website or in future promotional publications and presentations. No names of students will be associated with any of the photographs. When a student commits to Cavalcade of Authors West, the family or legal guardian is granting permission for the student's image to potentially appear in future publications.

Reminders:
--> Up to two chaperones are admitted free.
--> Standard permission forms that are required by your school/district must be completed for this trip and be kept on file by the school.
--> Payment must be received by February 1, 2020. Student workshop registration must be completed by Friday, March 20, 2020.

Please note:
~Registration is first come, first serve based on payment received. If there is a problem or concern, please contact Carrie Willenbring – see contact information below.

~Purchase orders are welcome, but final payment must be paid by February 1, 2020.

~Student participants will not be able to register for workshops until payment is made in full for the school. Workshops will fill first come, first serve, and will close once full. Once Cavalcade of Authors West receives your school's payment in full, Carrie Willenbring will email the school representative with the link for students to register for workshops.

Please indicate how you will be paying for Cavalcade 2020 Registration. **If someone other than the librarian makes a credit card payment, please send an email to coawest@gmail.com indicating the username of the payer so that we can match it with the appropriate school.
Cancellation Policy
If your school needs to cancel (as a school group), cancellation must be made 30 days prior to the event in order to receive a full refund. Cancellation must be in writing to coawest@gmail.com. If cancellation is made under 30 days, then the school will forfeit the entire registration amount. Your typed name below serves as an electronic signature that agrees to our cancellation policy.
