If you are interested in adopting, please fill out this questionnaire and send it back to me in either message form or in a document by copying the following questions and your answers.

Please be honest on this questionnaire with your answers! If you are unsure about the answer to a question please research the topic and if you cannot the information you are looking for feel free to ask. This questionnaire is to help further your knowledge in rattie care so everyone can be as happy and informed as possible.
