AUDIO VISUAL REQUESTS FORM
Please note the following:
1. Kindly submit a new request for each course.

2. All changes should be made 24 hours before requested date

3. The technician on duty should be informed if you exit the classroom before the allocated time frame ends.

4. All persons requiring this service are advised that classes must be completed by at least 5 minutes to the hour to allow for set up or dismantling of the apparatus provided.
