We are a small, all-volunteer organization, so please do not submit an application if you are not serious about adopting/ready to adopt in the next few weeks.  We put time and energy into reviewing every application and going through our process to ensure we match our foster cats with the right home.

- I agree to care for my cat in a humane and responsible manner, and to provide it with a safe, clean, and comfortable living environment, and with nutritious food, fresh water, and attention.

- I understand that I must pay the non-refundable adoption fee to F.A.C.T. prior to adopting this pet.

- I agree to notify F.A.C.T. if any of my contact information changes.

- I understand that F.A.C.T. has the right to decline my application for any reason.

- I am adopting this cat as a permanent member of my family, and I agree to keep this cat in my care for the rest of its life. If for any reason I cannot keep my cat, I will immediately contact F.A.C.T. to discuss alternative arrangements. I will not abandon or relinquish my cat to any other shelter, society, or person.

- I agree to provide my cat, at my own expense, with regular care by a licensed veterinarian, including an annual wellness examination, rabies vaccinations, other vaccinations as recommended by my veterinarian, and immediate care should the cat become sick or injured.

- I understand that my cat has been carefully examined by a veterinarian and has been provided with all age- appropriate vaccinations and other preventive care as noted on the cat’s health record.

- I understand that any medical conditions and/or behavioral issues detected while in the care of F.A.C.T. have been noted on the cat’s health record and disclosed to me prior to adoption. I understand that F.A.C.T. cannot guarantee or make any warranties regarding the health, behavior, or temperament of the cat(s).

- I understand and agree that F.A.C.T. is not responsible or liable if an animal in the home becomes ill and requires veterinary care after the adopted animal has been placed in the home.

- I give consent to F.A.CT. to contact my veterinarian about any information pertaining to this pet.

- I understand that it is best practice to separate a newly adopted pet from other pets in the home.

- I understand that it may take time for a newly adopted pet to settle in to a new environment.

I hereby accept possession of and responsibility for the pet(s) I adopt, subject to the conditions of this contract, and hereby release and discharge F.A.C.T. (Feline Advocates Coming Together), Inc. from liability for any injury or damages to any person or property caused in the future by said animal, and from any causes of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.

I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with F.A.C.T. (Feline Advocates Coming Together), Inc.

If we do not call or email you within 5 days of receiving your application, it means that we do not have a good match for you at this time.  However, we will keep your application on file and contact you if a good match comes into our care.
