Samoyed - Report a Health Condition
Please use this form (one form per dog) to record details of any health conditions diagnosed by a qualified veterinary practitioner in a Samoyed owned by you, wherever in the world you reside. Instructions on how to fill out the form are given in the next section: please read these before proceeding.

Behavioural conditions not diagnosed by a vet can also be reported.

There is an additional section for any conditions you believe your Samoyed is affected by that have not yet been diagnosed by a veterinarian.

Please do not use this form if your dog is a Samoyed crossed with any other breed.  

By notifying the Breed Health Coordinator of a health condition here, you give your consent for the data to be published anonymously or used in research for the benefit of breed health improvement. All information provided will be treated in the strictest confidence, and no identifying information will ever be published.
