SBAC Password Reset Request Form

Steps to take if you are not receiving your SBAC password: (1) check your SPAM/Trash folder; (2) ask your Test Site Coordinator to delete and re-add your name/email address; (3) If you followed steps 1 and 2 and are still not receiving the password from a Smarter Balanced email notification, please complete the information below and Student Assessment will contact SBAC daily at 2pm to have the issue resolved.
