Due to the COVID-19 school closure, parents may be eligible to receive a refund of prepaid childcare fees from the district's childcare centers. Please complete the following form to request a refund, apply refund to current or future fees, transfer your refund to another student's account or donate it to the program. Refunds may take 2-3 weeks pending confirmation of payment source. Fees paid by a third party will be reviewed on a case by case basis. A district representative will call to confirm receipt and processing of this form.
