Please fill out this form as your yearbook order form. If you have filled out and turned in a yearbook order form with a payment BEFORE school transitioned to remote learning, you do NOT need to fill out this form. Your order is already documented. If you are unsure if you have already ordered a yearbook, please call the school office at 402-715-7150 before filling out this form and sending in a payment.  

Once this form is filled out, you can then send a check made out to Upchurch Elementary School for $25.00 for each yearbook ordered to the school via USPS. Please note the child(ren)'s name and Yearbook in the memo line of the check. The school's address is 8686 S. 165th Street Omaha, NE 68136. The school building is closed to the public, please do not attempt to drop off your check at the school. Your check will be processed once staff returns to the building.

If your payment is received on or before June 5th, your order will be eligible for the June 12th Yearbook Pick Up. If your payment is received after June 5th, your order will be filled in the fall when school starts. If payment is not received, your order will not be filled.
