February Youth Lock-in at FUMC Westfield
This form will serve as the permission form to be submitted by parent/guardian of any youth planning to attend the October Youth Lock-in scheduled for Saturday, February 18th through Sunday February 19th.  The event will take place at FUMC Westfield, located at 1 E Broad Street, Westfield NJ.  Lead coordinator and chaperone for the event will be Marcee Rogers (908-591-6680 cell).  Additional chaperones who are background checked as per the church Safe Sanctuary Policy will also be present.  That full list will be provided to families at drop off on 2/18.

Note: Youth can attend just for Saturday night or can sleep over in the church.  Indicate the time parameters for your youth participant in the body of the form.  The full event runs from 6:30pm on 2/18/2023 through 11:00am on 2/19/2023. Saturday dinner, snacks and Sunday breakfast will be provided.  The cost for this event is covered through the youth ministry budget of FUMC Westfield.  Any donations toward the event will go to support Family Promise of Union County in connection with the supplies drive mission hosted this month by the Social Justice team.
