If you are involved in a near-miss incident or an accident (whether anyone is injured or not) while at the rowing club, either on or off water, you are asked to fill in this form within 24 hours. The purpose of collecting this information is that it  increases the safety awareness of everyone in the club and contributes to a safe rowing environment. Incidents and observations keep us alert and aware of further potential problems which we can then address. Collecting information allows us to check that we are doing better and not worse.
