Please complete this form if you need to change your child’s dismissal/transportation as a car rider for today only.

• This form will be checked daily until 2:00 p.m. (or 11:00 a.m. on early dismissal days). Changes submitted after that time cannot be guaranteed.
• This form is for same-day changes only (not for future dates).
• Bus changes apply only to students already assigned to that bus route (for example, riding a different assigned bus home instead of daycare or getting off at a different stop). This form cannot be used for a child not assigned to a bus to ride with another student.
• If you have multiple children, please complete a separate form submission for each child.
• This form replaces email or paper notes, as dismissal changes must be in writing (see Student Handbook, p. 6). Since notes can be lost in bookbags, we ask that this be the only method used for dismissal changes.  
• The office will notify your child(ren)’s teacher(s) once the form is received.
• Do not use this form for appointments during the school day.
