PHS Volunteer Availability Form, Fall 2019
Each PHS member household is expected to contribute 6 volunteer hours at PHS in the fall semester. Please mark all dates and shifts that you ARE available to volunteer at PHS in Fall 2019. A team of volunteers will assign shifts and email you your assignment of six (6) total volunteer hours for Fall 2019 by mid-summer.

We understand that plans sometimes change, so if you have to reschedule a shift, please post on the forum to request a swap with another member. If you are short on volunteer hours as the end of the semester nears, please get in touch at planethomeschoolmn@gmail.com to see how you can make up the difference and fulfill your requirement.

You must pay your membership fee, submit signed membership agreements and waiver, and submit this form to be eligible to register for classes at PHS. Emailing scanned PDFs of signed membership documents to planethomeschoolmn@gmail.com is acceptable.

Once all forms have been submitted and membership fee paid, members will be eligible to register as soon as volunteers manually "unpark" them and approve them for registration and other membership privileges.
