I give New Creation Dance Company and its members permission to take pictures/videos of myself/my dependent during New Creation dance activities (including dance classes, workshops, fundraisers, rehearsals, and performances), and also to use those pictures/videos for promotional purposes on their website, social media pages, fliers, or for any other form of advertising in the future. (Dancers will not be identified by name in any corresponding caption)

I understand that dancing has associated risks, including that of physical injury. I therefore agree to assume all such risks, and exempt, release, and indemnify New Creation Dance Company and its members from any liability loss or injury to me/my dependent or my (dependent’s) personal property which may occur due to any cause in relation to participation in New Creation activities (including dance classes, workshops, fundraisers, rehearsals, and performances). *
