MCV Membership Renewal 2025-26
Renewals due by June 30th.

Each *ACTIVE* volunteer must complete their own individual renewal form AND sign an Authorized Volunteer Service Agreement (AVSA). This is an annual requirement with Boise State University (BSU). Once you've filled out this application and paid online,  you'll receive an email message to sign the AVSA. You DO NOT need to sign this form if you are an Emeritus member.
