As an Altar Server for Our Lady of Lourdes Church, I understand that it is my responsibility to fulfill my duties and obligations to my God, my pastor, my parochial vicar, visiting priests, and our Church parish. I agree to arrive at Church 15 minutes before my assigned Mass in order to properly prepare, both physically and spiritually, to serve God. I agree to arrive wearing proper attire, including long slacks and dress shoes. I understand that no tennis shoes, flip-flops, or crocs are allowed. If I am unable to serve as scheduled, I understand that it is my duty to obtain a substitute and to notify my team captain. I agree and promise to carry out my duties to the fullest of my abilities and to always serve in the most reverent way possible.
As a parent/guardian of an altar server, I understand that I am responsible to assist my child with his/her/their commitment to serve at Our Lady of Lourdes Church. I agree to help him/her/them arrive at Church 15 minutes before the assigned Mass. I agree to ensure that my child will wear the proper attire when assigned to serve. I further understand that my child may be called on to serve at funerals and special events, including Midnight Mass and various liturgical services during Lent, Holy Week, and other special occasions.
