Perry Central School Digital Access Survey
The importance of access to devices and internet has been highlighted by the COVID-19 pandemic. At Perry Central School, we anticipate this access will remain necessary in the future, and recognize that digital equity is essential for educational equity.

To help serve our community, we ask that families complete an annual digital access survey for each child in your household currently enrolled at the Perry Central School District (any K-12 or “ungraded” student). This survey will help us maintain an updated, accurate and complete picture of the state of digital equity for each student in our district.

Thank you for taking time to complete this survey.
